Who we are
AppealCheck (the "Service") is operated by LIFEWORKS GROUP LLC ("we", "us"), a limited liability company registered in New Mexico, United States, New Mexico Business ID 7715331. Address: 8206 Louisiana Blvd NE, Albuquerque, NM 87113, United States. Email: admin@lifeworksgroupllc.com.
By using the Service, you agree to these Terms.
What AppealCheck does
AppealCheck is a self-service software tool. It reads a health insurance denial letter or Explanation of Benefits that you provide, on your own device, explains the reason and the usual appeal deadline using public rules, and prepares an appeal letter and a phone script that you can send or use yourself.
What AppealCheck is not
AppealCheck is not a law firm, a doctor, an insurance company or a patient advocate, and it does not give legal or medical advice. We do not contact your insurer or your doctor for you, and we do not promise any outcome. Your insurer decides after your appeal.
Your responsibilities
You decide what to send. Check that the letter matches your situation, ask your doctor for the medical documents, keep copies, and respect the deadline printed on your letter.
Price and payment
Checking a letter is free. The appeal letter and the phone script cost $49 (US dollars) per denial, paid once. The price is shown before you pay. There is no subscription.
Payments are processed by our payment providers. When a reseller acts as the merchant of record for your order, that reseller is the seller of record and its terms of sale also apply to the payment.
Delivery
The letter and the phone script are digital. They appear on your screen as soon as your payment is confirmed. Nothing is shipped.
Refunds
You can get a full refund within 30 days of your purchase. See our Refund and Delivery Policy.
Accuracy of the information
AppealCheck explains common denial reasons and deadlines from public sources such as HealthCare.gov and Medicare.gov. Your plan's own rules and the deadline printed on your letter come first. The tool can misread a photo: always compare what it read with your letter. The Service and its information are provided "as is".
Limitation of liability
To the extent permitted by law, our total liability for any claim related to the Service is limited to the amount you paid us for the order concerned, and we are not liable for indirect or consequential damages.
Intellectual property
The Service, its software and its texts belong to LIFEWORKS GROUP LLC. The letters prepared for you are yours to use.
Governing law
These Terms are governed by the laws of the State of New Mexico, United States, without regard to its conflict of law rules.
Changes and contact
We may update these Terms. The date at the top of this page shows the current version. Questions: admin@lifeworksgroupllc.com.
